In this meta-analysis of 75 randomized controlled trials in 1,823 men and women, supplementation with nitrate decreased systolic and diastolic blood pressure and improved markers of vascular function.

The study
The participants included adults without known health conditions (42 trials) and participants with hypertension/prehypertension (19 trials). In the vast majority of the trials, nitrate was administered in the form of beetroot juice, usually compared to a placebo like nitrate-depleted beetroot juice.
The daily dose of supplemental nitrate ranged from 0.35 to 27.84 mmol (22 to 1,728 mg). The intervention duration ranged from 30 minutes to 91 days.
The outcomes were assessed immediately (within a few hours of supplementation), in the short term (1–7 days of supplementation), and in the medium term (more than 7 days of supplementation).

Supplementation with nitrate increased both plasma nitrate and nitrite in the short term (low or very low certainty) and longer term (moderate certainty).
In the subgroup analyses:
- Each 70-mL increase in beetroot juice intake reduced systolic blood pressure by 1.2 mmHg in the short term and diastolic blood pressure by 0.6 mmHg in the short term.
- Beetroot juice reduced systolic blood pressure in the short term and medium term, and the effect plateaued at approximately 100 mL in the short term (–2.7 mmHg) and about 150 mL per day in the medium term (–4.56 mmHg).
Supplementation with nitrate did not increase the risk of side effects or withdrawal from the intervention.
